Job Summary And Responsibilities As our Social Worker at Chandler Regional Medical Center, you’ll perform social work assessments and interventions for inpatients, outpatients, and emergency department patients. Working collaboratively with all healthcare team members, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community. Responsibilities Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at‑risk populations Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families Serve on hospital committees and develop and present in‑services to hospital staff Job Requirements This is a temporary role. Position will be evaluated at the 1 year mark. Minimum 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting LMSW license from the AZ Board of Behavioral Health Examiners Master's Degree from a School of Social work accredited by the Council of Social Work Education Ability to work independently as needed with a high degree of detail orientation Ability to work efficiently in a fast‑paced environment with changing priorities Ability to work collaboratively with an interdisciplinary care team Preferred Minimum 3-Year Post-MSW healthcare experience Where You'll Work Chandler, Arizona, has a stable suburban population with an economy anchored by many large financial, and high tech companies.
